Who the Hell Signed This?

I have been cleaning out my office for the past week and I have begun to find a lot of random stuff. Anything from old entries, cheese balls, etc.

One random thing I found was this though, and it's been annoying me ever since I found it.

Who or what signed it? Is this an album cover? Is it even a rock artist? Is it something from one of our sister stations of Channel 957, 100.5 The River, or Greatest Hits 98.7 WFGR?

I don't know, so I need your help.
